Thank you for your interest in Ontario ARC! We are looking forward to meeting you and getting you started in the services eligibility process.

To help prepare for your intake meeting, please gather and bring with you the following documents:

  • Individualized Service Plan (most recent – within 12 months)
  • Complete Physical Exam (within the past 12 months; including list of medications)
  • PPD/TB test (within the past 12 months)
  • Chicken Pox/ Varicella immunization or written results of the Varicella titer
  • DDP-2
  • DDP-4 (day service needs)
  • Social History
  • Psychological Evaluation
  • Copies of Social Security, Medicaid and Private Insurance cards
  • Notice of Decision
  • Level of Care Eligibility

In addition, please bring the following documents if applicable:

  • Individualized Education Plan (most recent)
  • Psychiatric Evaluation
  • Current Behavior Plan
  • DDSO Risk Assessment
  • Program Plans and IPOP
  • DVE/CWBA (if applying for a vocational program)
  • Clinical evaluations
  • Legal Guardian Paperwork
  • Current SSI/SSD award letter
  • Community Habilitation Authorization Forms

The Intake Department will accept the most recent assessments available for the purposes of intake only. However, the agency will require the most current assessments before enrollment in any Arc program.
